Anyone have a list of Yankees Memorial bands, numbers, patches?

I think someone might of listed one before, but I can't find it. I'm looking for a list of all the memorial armbands, numbers, and anniversary and commemorative patches that have been placed on Yankees jerseys over the years. Off the top of my head I know:

1979 - Armband (Munson)
1990 - "1" (Martin)
1994 - MLB Anniversary Patch?
1995 - "7" (Mantle) and Armband (?)
1996 - Armband (?)
1997 - Jackie Robinson Patch
1999 - "5" (DiMaggio) and Armband (?)
2000 - Armband (Bob Lemon?)
2001 - ? Anniversary Patch
2003 - 100th Anniversary Patch
2007 - "10" (Rizutto) and Armband (Lidle)
2008 - Armband (Murcer), Allstar Patch, and Final Season Patch
