The inscription and autograph are legit, here's a photo of a 2003 mitt signed by David Ortiz. I notice that Ortiz signs his items with "Game Use" instead of "Game Used"

It looks like the hologram on the cap is from ASI (Authentic Sports Investments), Ortiz sold a lot of his game used items to ASI in the past.
