I have a pair of Nike GU cleats which exhibit the below tagging/labels....Can anyone here help me decipher what information (other than the size) is really given here (i.e. what does Promo Sample mean)? Thanks!

My knowledge is limited but seems this was used in 2006 season and Promo Sample just means it was not available to the public (retail).

Probably a basketball shoe collector can tell you more. Will view again to learn what they know.

The tag on top is a retail tag. The tag on the bottom is what you're looking for in terms of exclusivity. All the numbering isn't going to mean much for collectors. From what I gather is the obvious information. Huarache 2K5 cleat, size 13, blk/wht/silver, 2006 season and made exclusively for a team or player.

Promo Samples are special make ups usually not offered to the public. That normally means the cleat/shoes are made in a colorway specific to the team/player. However, a shoe/cleat can have a promo/sample tag without being visual different than a retail model. In these cases, the inner tag (shown here) would be the only indicator that it is something unique.
