Ademola Okulaja North Carolina Tarheels practice worn reversible jersey - he played from 1995-1999 at UNC and then played 10 years professionally overseas. Crazy that this wound up just outside of Nashville, but I'll take it.

Stockton Ports Home (worn by Landon Powell, now the A's back-up catcher, caught Dallas Braden's perfect game)

High Desert Mavericks (haven't identified the wearer yet)
Lake Elsinore Storm Alternate (worn by Matt Hagen)
Lake Elsinore Storm Home (worn by Dirk Hayhurst, played for the Padres and Blue Jays, author of the book "The Bullpen Gospels"

That is the Lake Elsinore Storm. They are a Padres single A squad.

That is the Lake Elsinore Storm. They are a Padres single A squad.

They also won the California League championship this past season. Their cap is one of, if not the most popular cap in all of professional baseball.
