All the Jordan jerseys from the Washington Wizards years that I handled were autographed and came with a Upper Deck Authenticated holo and COA.

I'm very skeptical of any jerseys with just the Wizards COA.

Yes it does come autographed with upper deck coa.

Does the UDA documentation state that the jersey is game used?

If you have that then you have what the collectors want. Keep in mind while Bulls jerseys go for $10,000+ with great documentation, the Wizards jerseys don't get as much but you can still find people willing to pay $7,500 to $15,000 in the right type of auction. The upper end with a photomatch.
