You've got it pretty well covered. The flag caps were worn from 2001-07...then the specially designed caps were introduced in 2008. Teams only wore them on Memorial Day, July 4th and September 11th, except for 2001, when they were worn after the resumption of play following the 9/11 terrorist attacks for every game for the rest of the season.

All I can guarantee you is that the NY Mets wore te caps with the flag on the side until the year 2008 when it started I don't know for sure. I would assume other teams followed the same trend as the Mets in 2008, but I'm not positive and haven't dug into it too much.

The Stars & Stripes caps as they are designed now first appeared in 2008 with dark blue lids. The caps were red in 2009 and white crowned in 2010.
Prior to that were the Flag caps (with US flag logo on the left side). The 2002-07 versions were pretty much done the same way, and worn for 4th of July and 9/11.
The original flag caps were introduced immediately after MLB resumed from the period of mourning for the 9/11 terrorist attacks. These were made with the flag directly embroidered into the cap fabric, and worn exclusively from the resumption of play through season's end.
The 2002-07 versions had the flag as a patch that was heat pressed onto the cap fabric.
I have one of the 2001 versions in my collection, a Kevin Young Pirates which KY gave to me at the last home game at Wrigley Field in 2001.
